• Collaboration Agreement (regarding medication evaluation and management by Dr. Sterling in collaboration with the client's primary therapist)

• Medicare Private Contract (If you are a Medicare or Medicaid recipient, this should be read and signed by you or the person who has power of attorney for your financial affairs)

• Authorization for Dr. Sterling to Release Confidential Information to Others
